Yoga for Wellbeing Feedback Form

Thank you for taking part in our Yoga for Wellbeing course. I really hope this course has been of benefit to you and I would appreciate you taking a moment to share your feedback and comments. This helps us show funders what we have achieved but also helps us to shape and create future offerings. So please do be honest.


If you feel comfortable sharing your name this helps us compare answers from your original application form with how you feel now but if you would prefer this to be anonymous that is also ok.

When you first applied for the course you were asked to rate how you were feeling in the following areas. Please rate them again based on how you are feeling now, having done the course. Don't worry if you can't remember your previous answers.
